A "tool kit" or roadmap that explores a range of signposts in life's journey, which will help people consider how they will age well at home now and into the future. The roadmap gives information and practical provisions to progress along the individuals unique path. It offers support and allows people to take control, explore and consider what is important to them and how they might get there.
